What are dental fillings? What dental materials are used for fillings?
Dental filling is a restorative dental method that uses special materials (alloy or non-metal) to rebuild lost tooth tissue in order to restore function and aesthetics for the patient.
Fillings can be done directly or indirectly. Some materials used for fillings:
Direct: The doctor shapes the filling directly in the patient's mouth without taking impressions to send to a lab. Treatment is completed in one appointment.
Amalgam: Silver in color, an alloy of mercury and other metals such as silver, copper and tin. Amalgam expands over time but is no longer used today because mercury is a heavy metal.
Composite: Colored close to real teeth, a resin compound with quartz, silica and glass fillers. Composite shrinks over time, so flowable and dense composite are often used together, layered, to minimize voids after filling and limit composite shrinkage.
GIC: Colored like real teeth, a compound of glass silica powder.
Indirect: The doctor takes the patient's dental impressions, sends them to a lab for fabrication and then bonds the restoration onto the tooth with special adhesive.
Composite
Porcelain inlay/onlay/overlay: Indirect restorations commonly used in place of composite when too much tooth tissue is lost to be filled tightly and durably with composite but the patient does not want a porcelain crown.
- Gold: Metallic gold in color, usually 18K gold (sometimes 24K gold is used but rarely, since 24K gold is too soft and struggles to keep its shape under chewing forces).
- Glass ceramic (Emax or Lisi): Colored like real teeth, both aesthetic and hard enough to withstand the patient's chewing forces.
When are fillings needed?
Decayed teeth with no exposed pulp.
Cervical tooth wear.
Chipped teeth.
Gapped teeth.
Recurrent decay under an old filling that needs refilling.
Benefits of fillings?
Completely resolves tooth decay.
Restores function and aesthetics for the patient.
Minimally invasive, preserving as much tooth tissue as possible.
High hardness ensures it withstands chewing forces during use.
Filling procedure?
Direct
Step 1: The doctor examines, plans treatment and scales the teeth before preparing the cavity.
Step 2: The doctor prepares the cavity, removes damaged dentin, and cleans and treats the dentin before filling. If the patient experiences sensitivity, anesthesia may be needed.
Step 3: The doctor applies adhesive to the cavity + uses instruments to pack and shape the filling to the tooth's anatomical form and cures it with a light.
Step 4: The doctor checks the bite and grinds to adjust and finish the filling. The filling is then polished.
Indirect
Step 1: The doctor completely cleans out the cavity + polishes the enamel and dentin surface before taking a rubber impression to send to the lab.
Step 2: The technician pours the model and fabricates the filling in porcelain/composite/gold and sends it back to the doctor.
Step 3: The doctor checks the fit of the filling and bonds it onto the patient's tooth with special adhesive.
Step 4: The doctor checks the bite and grinds to adjust and finish the filling. The filling is then polished.
Possible complications of fillings?
Some complications that may occur after fillings are:
Dull ache: This is a sign that the cavity has reached the pulp and root canal treatment is needed.
Pain when biting the two arches together: This may be due to a high spot; you need to go to the dentist so the doctor can adjust the bite again.
Pain from sensitivity to hot and cold: This shows the pulp nerve is damaged, and root canal treatment or extraction may be needed.
